DISCLAIMER

This Disclaimer explains the scope and limitations of Matrix Functional Performance website content, communications, educational materials, and general information.

EFFECTIVE DATE · SEPTEMBER 7, 2026    |    LAST UPDATED · SEPTEMBER 7, 2026

This Disclaimer applies to the website, content, materials, communications, and general information made available by Matrix Functional Performance, LLC (“Matrix,” “we,” “us,” or “our”).

By accessing or using the Matrix website, you acknowledge and agree to the limitations described below.

01NON-CLINICAL SERVICES

Matrix Functional Performance provides non-clinical educational, functional nutrition, lifestyle, and performance coaching services.

Matrix services are intended to help individuals better understand and manage factors related to nutrition, lifestyle, training, recovery, behavior, and performance.

Matrix does not provide medical care through its website or general educational materials and does not represent such information as medical diagnosis or treatment.

Matrix does not diagnose, treat, prevent, or cure disease or medical conditions.

02NOT MEDICAL ADVICE

Information provided through the Matrix website, educational materials, articles, communications, application process, or other general resources is provided for educational and informational purposes only.

Nothing provided through these channels should be interpreted as medical advice; a medical diagnosis; medical treatment; a prescription; emergency medical guidance; or a substitute for individualized evaluation or care from an appropriately licensed healthcare professional.

You should consult an appropriately qualified healthcare professional regarding medical conditions, symptoms, diagnoses, medications, treatments, injuries, or other matters requiring medical evaluation or treatment.

03NO HEALTHCARE PROFESSIONAL-PATIENT RELATIONSHIP

Accessing the Matrix website, reading Matrix content, communicating with Matrix, submitting a Private Client Application, or participating in a Discovery Call does not establish a physician-patient, healthcare professional-patient, or other licensed clinical relationship.

Submitting an application also does not establish a Matrix client relationship.

A Matrix client relationship is established only after an applicant has been accepted and the applicable engagement process and agreements have been completed.

04MEDICAL EMERGENCIES

The Matrix website, application, email addresses, communications, and coaching services are not intended for emergency medical care and should not be used to seek emergency assistance.

If you believe you are experiencing a medical emergency, call 911 or seek appropriate emergency medical assistance immediately.

Do not delay emergency or professional medical care because of information provided by Matrix.

05FUNCTIONAL NUTRITION AND LIFESTYLE INFORMATION

Matrix may provide educational information concerning nutrition, food choices, hydration, sleep, recovery, stress management, lifestyle behaviors, training, and other factors that may influence performance and human capacity.

Such information is educational and should be considered within the context of an individual's circumstances.

Individual nutritional and lifestyle needs can vary based on numerous factors, including health status, medications, medical conditions, allergies, intolerances, physical demands, training status, age, and other individual considerations.

When medical evaluation or treatment is appropriate, clients should work with an appropriately licensed healthcare professional.

06SUPPLEMENTS

Matrix may discuss dietary supplements, nutrients, or related products for educational purposes when appropriate within the scope of its services.

Such discussion does not constitute a prescription or medical treatment.

Dietary supplements may carry risks, may interact with medications or medical conditions, and may not be appropriate for every individual.

You are responsible for discussing supplements with an appropriately qualified healthcare professional when your health status, medications, medical conditions, pregnancy, allergies, or other circumstances make professional medical guidance appropriate.

Matrix does not guarantee the safety, purity, effectiveness, manufacturing quality, or regulatory compliance of products manufactured or supplied by third parties.

07EXERCISE, TRAINING, AND PHYSICAL ACTIVITY

Exercise and physical activity involve inherent risks, including the possibility of injury.

Information concerning exercise, training, physical conditioning, recovery, or performance should be applied appropriately to the individual's abilities, experience, physical condition, and circumstances.

Before beginning or materially changing an exercise or training program, individuals should seek appropriate professional evaluation when medical conditions, injuries, symptoms, physical limitations, or other circumstances make such evaluation advisable.

Stop physical activity and seek appropriate medical attention if you experience symptoms or circumstances requiring medical evaluation.

08BIOMETRICS, ASSESSMENTS, AND PERFORMANCE DATA

Matrix may use or discuss information derived from assessments, questionnaires, wearable devices, laboratory information provided by a client, biometric data, performance measurements, training data, lifestyle information, or other sources.

Such information may help identify patterns, trends, relationships, or areas for further consideration within Matrix's educational and performance-coaching framework.

Data does not, by itself, constitute a medical diagnosis.

Matrix's interpretation or discussion of performance, lifestyle, biometric, or other data within its non-clinical scope should not be interpreted as medical diagnosis, medical treatment, or a substitute for appropriate clinical evaluation.

Measurements and data may also be incomplete, inaccurate, affected by device limitations, affected by collection conditions, or subject to interpretation.

09LABORATORY AND OTHER THIRD-PARTY INFORMATION

Clients may choose to provide Matrix with laboratory results, medical records, wearable data, reports, or other information obtained from third parties.

Matrix's use or discussion of such information is limited to its applicable non-clinical educational and performance-coaching scope.

Matrix does not independently guarantee the accuracy, completeness, methodology, interpretation, or reliability of information produced by third-party laboratories, devices, healthcare providers, technology platforms, or other sources.

Clinical interpretation, diagnosis, and medical treatment decisions should be made by appropriately licensed healthcare professionals.

10NO GUARANTEE OF RESULTS

Human performance and health-related outcomes are influenced by numerous factors and vary substantially among individuals.

Matrix does not guarantee any particular result, including improvements in health; symptoms; body composition; metabolism; energy; sleep; recovery; strength; endurance; athletic performance; cognitive performance; professional performance; or any other health, fitness, lifestyle, or performance outcome.

Results may depend on individual circumstances, consistency, adherence, health status, environment, training, nutrition, recovery, genetics, medical conditions, and factors outside Matrix's control.

Statements regarding potential benefits or outcomes should not be interpreted as guarantees.

11TESTIMONIALS, EXAMPLES, AND SUCCESS STORIES

Testimonials, examples, case discussions, success stories, or descriptions of individual experiences may illustrate what a particular person experienced.

They are not guarantees or representations that another individual will achieve the same or similar results.

Individual experiences vary.

Where applicable, Matrix will seek to present testimonials and endorsements in a manner consistent with applicable advertising requirements.

12EDUCATIONAL CONTENT AND INSIGHTS

Articles, Insights, social-media content, presentations, newsletters, videos, educational resources, and other Matrix content may discuss scientific research, emerging concepts, performance strategies, nutrition, physiology, lifestyle, or related subjects.

Scientific knowledge and professional understanding evolve over time.

Matrix may update its views, methodologies, recommendations, or educational content as new information becomes available.

General educational content cannot account for every individual's circumstances and should not be treated as individualized medical guidance.

13THIRD-PARTY PRODUCTS, SERVICES, AND INFORMATION

Matrix may reference or use third-party products, technologies, laboratories, devices, software, research, services, or educational resources.

Unless expressly stated otherwise, reference to a third-party product or service does not constitute a guarantee or warranty by Matrix.

Matrix is not responsible for changes to third-party products, services, information, availability, security, accuracy, or performance outside Matrix's control.

Any financial relationship, sponsorship, affiliate arrangement, or other material connection relevant to an endorsement or recommendation should be disclosed where required by applicable law.

14PROFESSIONAL COLLABORATION

Matrix services are designed to operate within Matrix's defined non-clinical scope and may complement—but do not replace—appropriate services provided by physicians, registered dietitians, physical therapists, mental-health professionals, or other licensed professionals.

When circumstances fall outside Matrix's scope, Matrix may recommend that an individual seek evaluation or support from an appropriately qualified professional.

Such a recommendation does not itself create a professional relationship between the individual and any third party.

15PERSONAL RESPONSIBILITY

You are responsible for the decisions you make regarding your health, nutrition, lifestyle, training, and performance.

You are also responsible for providing accurate and relevant information when participating in Matrix services and for informing appropriate professionals of medical conditions, medications, symptoms, injuries, or other circumstances that may affect your decisions.

Matrix encourages informed decision-making and appropriate collaboration with qualified professionals when circumstances warrant it.

16WEBSITE AND GENERAL INFORMATION

While Matrix endeavors to provide useful, thoughtful, and current information, Matrix does not warrant that all website or educational content will always be complete, error-free, current, or appropriate for every individual.

Reliance on general information provided through the website is at your own discretion and should take into account the limitations described in this Disclaimer.

17RELATIONSHIP TO OTHER MATRIX AGREEMENTS

This Disclaimer should be read together with the Matrix Terms of Use and Privacy Policy.

Active clients may also be subject to separate engagement agreements, informed acknowledgments, program terms, or other documents governing Matrix services.

If a separate signed client agreement contains provisions specifically applicable to an active client engagement, that agreement will govern the client relationship to the extent provided in that agreement.

18CHANGES TO THIS DISCLAIMER

Matrix may update this Disclaimer periodically to reflect changes in its services, methodologies, business practices, or applicable requirements.

When changes are made, the Last Updated date above will be revised.
